SummaryThe Home & Supportive Care Certified Nursing Assistant (CNA) provides essential personal care and supportive services to patients in their homes or in care facilities across both home health and hospice settings. Working under the direction and supervision of a registered nurse (RN), the CNA assists with activities of daily living (ADLs), monitors patient condition, and ensures safety, comfort, and dignity for patients facing chronic illness, recovery, or end-of-life care. This role is central to promoting quality of life and compassionate support in a primarily home-based setting.Essential Duties and ResponsibilitiesThese duties and responsibilities described below represent the general tasks performed on a daily basis. Any other duties as needed to drive to the vision fulfill the mission and abide by the values of the organization.Personal Care & Support - Per Established Care PlansProvide hands-on assistance with bathing, grooming, dressing, toileting, oral care, and other hygiene needsAssist with safe transfers, mobility, positioning, and ambulation, including use of assistive devicesOffer support with nutritional needs including feeding assistance, and hydration remindersPromote patient dignity, independence, and emotional comfort during all care taskso Provide compassionate presence for hospice patients and their families, including at the end of lifeObservation & CommunicationMonitor and document changes in patient condition, behavior, or needsReport clinical changes or safety concerns promptly to the supervising nurseSupport timely and thorough documentation of visits and care provided, in accordance with organization policiesHome Environment & SafetyHelp maintain a clean, safe, and orderly environment for patient comfortPerform light housekeeping tasks related to patient care (e.g., linen changes, laundry, light cleaning)Promote infection control and follow universal precautions at all timesEnd-of-Life Care (Hospice Specific)Provide personal care and comfort measures to hospice patients in alignment with the care planRespect cultural and spiritual beliefs of patients and familiesSupport families and caregivers with emotional presence and practical assistanceRecognize signs of imminent death and notify the hospice nurse as neededWorking ConditionsFrequent travel to patient homes across the service areaExposure to diverse home environmentsIn hospice, may involve providing care after death and supporting the familyShowcase strong time management skills, enabling the ability to make impactful judgment calls on patient care while balancing all other responsibilities and time engagements.All Home & Supportive Care clinical staff function as integrated members of the broader service line team. They perform patient care encounters as assigned and may be temporarily reassigned to other areas within the service line, in accordance with policy.Attend required hospital-wide orientations, meetings, and in-servicesDemonstrate a commitment to flexible work scheduling when necessary to ensure patient careEducation and ExperienceMinimum Required Education: High school diploma, or equivalent.Preferred Education: Experience: At least 1 year of experience as a CNA in home health, hospice, or long-term care is preferredCertificates, Licenses, RegistrationsMust hold current certification as a certified nurse aide in Virginia.Must hold, or obtain within 90 days of hire, and maintain American Heart Association BLS Certification.Must hold and maintain a valid Driver's License, automotive insurance, and acceptable Motor Vehicle Record in compliance with organizational policy and/or demonstrate a reliable method of transportation for work responsibilities.
