Full-Time Clinical Care Coordinator

Job Summary

The Clinical Care Coordinator (RN) supports the Clinical Manager in overseeing clinical staff to maintain productivity, compliance, and job satisfaction. This role focuses on ensuring quality patient care, adherence to healthcare standards, and staff development, while fostering a safe environment for patients in their homes.

Key Responsibilities

• Supervise, guide, and develop nursing staff performance.
• Coordinate KPI updates with management and assist in quality assurance initiatives.
• Collaborate with teams to meet QAPI program goals and improve patient care.
• Educate clinical staff on policy updates, HHCAHPS scores, and best practices.
• Conduct field training for new patient care personnel.
• Provide case management and follow-up skilled nursing visits.
• Investigate and resolve patient care concerns.
• Participate in continuing education and share updates with the clinical team.
• Oversee documentation, staffing assignments, and compliance with regulations.
• Perform supervisory visits to assess training needs and patient satisfaction.
• Ensure proper use of supplies, equipment, and workplace efficiency.
• Serve as a role model, demonstrating professionalism and teamwork.
• Act as a backup administrator, if needed.

Qualifications

• Registered Nurse (RN) license in the state of practice (Required).
• Current CPR certification (Required).
• Basic computer skills.
• One year of home health care experience (Required).
• Experience in wound care, vacs care, ostomy care, and IV administration (Preferred).

Skills & Abilities

• Strong knowledge of OASIS-E1 documentation.
• Ability to work independently and make sound judgments.
• Solution-oriented, resilient, and able to navigate clinical challenges.
• Excellent interpersonal skills, with the ability to handle difficult situations professionally.
